What Is Accident, Sickness & Hospital Insurance?

Accident, Sickness & Hospital (ASH) plans—sometimes called fee-for-service plans—give you the freedom to choose your own doctors, specialists, and hospitals without requiring referrals or a primary care provider. The insurance company pays a set portion of your total charges, and coverage can often begin immediately, with guaranteed acceptance in many cases.

How Do These Plans Work?

With an ASH plan, you have the flexibility to direct your own care:

  • See any physician, specialist, or hospital you prefer

  • In many cases, receive coverage starting with your very first claim

  • Pay up front for care (in some plans) and submit claims for reimbursement

  • Benefit from first-dollar coverage, where the insurer begins paying from your first claim without requiring you to meet a deductible

In some situations, a deductible may apply. Once it’s met, the insurer generally covers a percentage of the usual, customary, and reasonable (UCR) rate—the standard amount healthcare providers charge in your area.
